In some depressed patients, physical symptoms may play a leading role and on occasion may be all that appears on the surface. The depressed mood may emerge only after careful questioning and examination.

Since the complaints are somatic in nature, they usually lead the patient to the family doctor, internist or surgeon who may often be baffled by the patient's fatigue and nonspecific nature. Fatigue, headaches, insomnia, back pain, vague discomfort in arms and legs, and various gastro intestinal complaints (such as feelings of fullness in the abdomen, upset stomach, belching, constipation) may make an underlying depression, which the patient will describe openly and with feeling when appropriately interviewed. All too often such people are given numerous tests and told that nothing shows up. "It's all in your head, so just forget it; get a hold of yourself; keep busy, take a trip," etc. This usually results in the person seeking another physician, usually with the same results.

This cycle keeps recurring much to the detriment of both patient and his bank account. If a person develops any of the aforementioned symptoms with or without some recognized precipitating event), and they persist over a period of several weeks or seem to get worse as time goes on, that person should consult a psychiatrist or another specialist in mental disorders. If a disorder were present requiring a surgeon or internist, wouldn't we contact one of them? Unfortunately, most general practitioners of medicine are so overworked that they simply do not have the time necessary to diagnose the "masked" depressions. As With any other illnesses, once the correct diagnosis is established, the treatment is relatively simple. There are many very effective antidepressant medications available.

When properly aclministered, these medications will relieve the symptoms of depression. Psychotherapy is also required in many cases. Some of the most severe forms of depression, such as the manic depressive or psychotic, may require a relatively short period of inpatient hospital care. Treating depression can be a great source of joy and satisfaction to both long suffering patients and the physician when, after a few weeks of appropriate treatment, that person begins to enjoy living again.
